Does the 837I format have a standardized location to send a facility’s CLIA number?
We are receiving denials from a state Medicaid with the understanding it is due to the CLIA number not being submitted with the claim. While we are familiar with the loops where it would be submitted within the 837P format, our understanding is that the institutional claim does not support it being sent.
The 837I claim form does not have a formal CLIA number segment or data element as the 837P does.
We would recommend reaching out to the specific payer sending the denial on how and where they are expecting the CLIA to be sent.
