Loop 2000D Not Required When Rejected at Information Receiver Level (277)
Description

Is the situational rule for Loop 2000D (Subscriber Level) HL segment incomplete? It reads as follows:

"Required when the patient is the subscriber or a dependent with a unique identification number and status was not reported at the Provider level. If not required by this implementation guide, do not send."

I think it would also not be required when status was reported at the information receiver level. If the payer populated Loop 2200B (Information Receiver Trace Identifier) STC01-01 with a rejection claim status category code such as "D0" then they would not have anything to put in Loop 2000D. This also aligns with Section 1.4.3.2 Status Response Levels of the front matter.

Scenario

A payer sent us a 277 transaction which contains a Loop 2200B STC segment but does not contain Loop 2000D.

RFI Response

In the hierarchical structure of the 277 transaction, reporting an STC segment at any level prevents the use of subsequent levels. The situational rule at Loop 2000D is considered complete because it references the preceding hierarchical level. Section 1.4.3.2 of the front matter accurately outlines the status response levels. 

In the business scenario above, the payer is correct not to send Loop 2000D.
